Launch of new myCMC - allowing patients to easily create their own urgent care plan

NHS to empower patients to record and share urgent and end of life care wishes.

NHS Coordinate My Care clinical service launches online ‘myCMC’ for the first time allowing patients to easily create their own urgent care plan, so they can receive the care they want, when and where they want it, and prevent inappropriate hospital admissions.

London – January 2018 – Patients across Greater London are to be given greater power to inform GPs, hospitals, ambulance crews, 111 providers, care homes, hospices and out of hours services, about their urgent and end of life care wishes, following the launch of a new online urgent care record called ‘myCMC’.
