The Coordinate My Care service is accessible via the GP IT system Vision using an ‘in-context link’.

What this means for CMC users is that, within Vision, you are able to create, edit and approve a CMC urgent care plan for your patients which will immediately be viewable by all the urgent care services including 111, the out of hours GP services and the London Ambulance Service.

What this means for Vision users is:

  • They can access CMC directly from Vision by either Consultation Manager or Apps Controller
  • In the next Vision release users will be automatically alerted to whether or not their patients already have CMC urgent care plans

What this means for Vision users is:

  • Users can access CMC directly from within Vision without having to login to CMC as a separate application
  • By entering their CMC username & password credentials just once, they do not need to enter these details again on each subsequent login to CMC
  • Vision users will automatically launch CMC in the context of their selected patient without having to search for and select the patient again
  • If a CMC care plan exists for the selected patient, they can view and update it
  • If a CMC care plan does not already exist for the selected patient, they can easily create one

The following materials are to support GPs in accessing CMC from Vision. These include a step-by-step guide for users so they can see what screens and prompts they will see when launching CMC from Vision and the functions they will be able to perform.
