You should receive a confirmation email from us shortly with a summary of the information you have provided. If you do not receive this email within 24 hours, please re-submit your UAF. Please note the confirmation email may fall into your Junk / Spam folder so please check here before re-submitting.
Your request will be processed and log in details should be sent to you within 72 hours. If you require clinical access urgently before the 72 hour timeframe, please forward your confirmation email to coordinatemycare@nhs.net with ‘Urgent’ in the title of the email.
