Responsibility to patient – consent

You will need to obtain verbal consent from your patient to start a CMC care plan.  There are three options available within the care plan:

1. The patient has capacity and has agreed to the creation of a care plan and the sharing of the information within it between healthcare professionals

2. The patient lacks capacity but has a Lasting Power of Attorney for health and welfare

3. This is a best interest decision involving the people who know them best such as family members and GPs.

Only health and social care professionals and limited administrative staff helping them, who have a legitimate relationship with the patient can see the care plan
