How Coordinate My Care works
Coordinate My Care(CMC) is an NHS service that coordinates urgent care for patients. It starts with you telling us about your personal wishes and preference. You tell us how you would like to be treated in an emergency and who we should contact on your behalf. CMC will then let your doctor or nurse know that your plan needs to be completed. Your doctor or nurse will add your diagnosis, medical details, resuscitation status, medications and recommendations for how to care for you in an emergency. Your doctor or nurse then approves your plan. Once it is approved it is immediately made into a secure, digital Urgent Care Plan. CMC then shares the plan with all the urgent care services including 111, out of hours GPs, the ambulance (in their vehicles) and the emergency departments. This way everyone is in the loop and knows how to care for you when you call for help. You are no longer a stranger to the urgent care services.
